The Ramsay Connect team is made up of a wide range of health professionals and support staff, dedicated to ensuring you receive the care you need in the comfort and safety of your own home.
Ramsay Connect Care Coordinators are experienced nurses or allied health professionals. The Care Coordinators provide the important link between the patient, their doctors and Ramsay hospitals. They oversee the implementation of each patient’s care plan so that the team can deliver the highest quality care as part of the transition from hospital to the home.
Our community-based teams deliver your individualised program and are made up of nursing and allied health professionals such as physiotherapists and occupational therapists who specialise in providing care in your home or community. Ramsay Connect also facilitates other services that may be needed such as domestic cleaning or meal delivery during the patient’s recovery and rehabilitation.
If the Ramsay Connect community-based team is unable to provide a service that you need, we can draw on our extensive network of external community-based providers.
All our community-based team undertake thorough credentialing and are required to meet stringent quality standards. All Ramsay Connect clinical care services are overseen by a doctor.
