Rehab at Home

The first days and weeks at home following a hospital stay can be difficult. Ramsay Connect is here to support you through this time with Rehabilitation at Home – giving you access to a range of rehabilitation and health care services from the privacy, comfort and safety of your own home.

How does Rehabilitation at Home work?

Ramsay Connect provides in-home rehabilitation, reconditioning and health care services for a wide range of medical conditions, including:

  • Joint replacements
  • Fractures
  • Mobility problems or falls
  • Neurological conditions
  • Respiratory conditions
  • Cardiac conditions

During your stay in hospital, your doctor or another member of your healthcare team can refer you to receive Rehabilitation at Home through Ramsay Connect.

A dedicated Ramsay Connect Care Coordinator will contact you to determine your eligibility for program funding and discuss your specific care needs. Your Care Coordinator will also liaise with your hospital staff or doctor to develop an individualised rehabilitation program for you, drawing on expertise from a range of allied health and supportive care professionals.

Your Rehabilitation at Home program may include:

  • Physiotherapy - To improve your strength, balance, mobility and safety in and around your home
  • Nursing - To help you with wound management and overall health assessment
  • Occupational therapy - To help you achieve independence and safety in activities of daily living, including dressing and showering
  • Personal care - To help with showering and dressing
  • Home care - For domestic support such as cleaning
  • Meals - Delivered to your home to help in the early days after leaving hospital

While the Rehabilitation at Home program does not arrange for a doctor to visit you in the home, your Care Coordinator will keep your doctor or surgeon informed of your progress throughout the program.
