This information is for healthcare professionals who would like to refer a patient to Ramsay Connect.
Your patient needs a referral from a doctor or healthcare professional at a hospital to access a Ramsay Connect program. Follow these steps to complete a referral. You can complete this process during a patient’s hospital stay or at pre-admission, if preferred.
STEP 1: DISCUSS RAMSAY CONNECT WITH YOUR PATIENT
Talk to your patient about the benefits of receiving care or rehabilitation in the comfort of their own home and discuss whether a Ramsay Connect program is a suitable option for them.
STEP 2: COMPLETE A REFERRAL FORM
Complete Hospital Care in the Home Referral Form or Rehabilitation at Home Referral Form,
If you are referring a patient for Hospital Care in the Home, please also refer to the Hospital at Home Discharge Checklist and complete a Medication Chart.
Then, email the completed PDF/s to email@example.com.

STEP 3: AWAIT CONFIRMATION
You should receive an email to acknowledge receipt of the referral within 2–4 hours. If you do not receive this email, please call 1800 799 732.
A Ramsay Connect Care Coordinator will then confirm your patient’s eligibility for funding with their health insurer and contact you to begin to develop a care plan.
While getting health fund approval will usually take less than 24 hours, the process can take up to 7 days, so it is important to send referrals through as early as clinically appropriate to allow for this to occur. The Care Coordinators will keep referrers and the patients informed throughout this process.
